Benefit Eligibility 


Unless stated otherwise by an asterisk (*), benefits are available to full and part-time employees (50% FTE or higher) who are regularly scheduled to work a minimum of 40 hours per pay period. Benefits are effective the first of the month coinciding with or following the date of hire. Premiums vary depending on type and level of coverage elected.

Medical

Medical*

Traditional medical plans and a High-Deductible Health Plan are available through SSM Health’s WellFirst Health network. All plans include a nationwide prescription drug benefit. Employees can receive free preventive health physicals through SSM Health's medical insurance. 
Dental

Dental*

SSM Health provides two dental plans from which to choose: Dental Plus or Dental Base. You may use any dentist you choose; however, using an in-network provider offers additional discounts and enhances the total services you may receive under the annual maximum benefit.
Vision

Vision

Two vision plans are available through VSP. The medical plan covers an annual eye exam. The plan assists with eye exams, lenses, frames and contact lenses. Included are copays for in-network coverage and reimbursement (up to plan limits) for out-of-network coverage.
Employee Assistance Program (EAP)

Employee Assistance Program (EAP)

SSM Health offers wellness services through the Employee Assistance Program (EAP). The program also offers free resources to both employees and eligible family members to help with financial planning, child care, marriage counseling, and other life events.
Discounted Medical Premiums

Discounted Medical Premiums*

Employees who meet certain household size and income guidelines and enroll in an eligible health plan may apply to receive up to a 100% discount on medical premiums and zero-cost prescriptions through waived pharmacy copays.
Legally Domiciled Adult

Legally Domiciled Adult*


​​​​​​​An employee can cover an adult household member on health insurance (and the dependent children of that LDA), if they are not covering a spouse on their plan. Examples would be domestic partners, a parent or an adult child.

Paid Time Off (PTO)*

All full-& part-time benefit eligible employees may accrue PTO on the first day of employment. PTO is used for vacations, holidays (New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, and Christmas), short-term illnesses, and other absences from work.

PTO Sell / Hardship*

Employees who accrue PTO may elect to sell (cash out) a portion of their PTO throughout the year. The Hardship program supports employees if they face a medical emergency or experience a qualifying hardship.

 Donate PTO to an emergency bank. This can be used by employees who have a medical emergency or experience a qualifying hardship.

Adoption Assistance*

After six months of employment, employees may be reimbursed for normal agency and legal fees incurred to adopt a minor child, up to the maximum benefit amount.

Paid Maternity Leave*

The Short-Term Disability benefit covers maternity leave. Employees will have 100% income replacement for up to six weeks for a routine delivery and up to eight weeks for a c-section.

Paid Parental Leave*

After 90 days of employment, employees are eligible for one week of paid parental leave for newborns or newly adopted children (pro-rated based on FTE).

Bereavement Leave*

Employees may take up to three scheduled shifts with pay in the event of the death of a family member; up to five scheduled shifts with pay are provided if the loss is a spouse or child/step-child.

FMLA

Eligible employees can take up to 12 weeks of unpaid, job protected leave for continuous, reduced work schedule, or intermittent time off for qualifying reasons.

You have the option to make any day a pay day! Our voluntary benefit offered through DailyPay provides eligible hourly team members instant access to their earned, unpaid base pay (fees may apply).

Employee Relief Fund*

Employees experiencing a financial hardship or crisis may receive assistance for events such as a serious illness or injury to themselves or immediate family members, loss of primary residence from fire or natural disasters, death of immediate family members, and more.

Retirement*

We are committed to helping employees enjoy their retirement through the following customizable retirement programs:

  • Employee Contribution Plan: This plan offers employees a method to invest their pre- and/or post-tax dollars to save for retirement income.
  • Employer Matching Contribution: Employees who participate in the voluntary employee contribution plan may be eligible to receive a match from SSM Health on a portion of their contribution.
  • Employer Basic / Fixed Contribution: Eligible employees receive an annual employer-funded basic contribution. Participation in the voluntary employee contribution plan is not required to receive the basic contribution. 

Additional retirement savings options are available at certain entities. More information will be provided upon acceptance of a job offer.
